Handle renamed authorization variables

Sometimes (when using rewriting with the workaround for CGI mode
described at
http://www.besthostratings.com/articles/http-auth-php-cgi.html) the
HTTP_AUTHORIZATION variable is renamed, this change detects this
renaming and uses the renamed variable.
