this adds an option to print the menu without icons (as we do in the
dokuwiki template) but icons were added nontheless

also adjusts styles and adds images

all menu items should have a default SVG. Template authors then can
decide if they want to use them or not.

These are only the page tool SVGs. More need to be added.

The images are now to be seen as deprecated but will remain for a little
longer. They will be released in the future.

The icon does not match the others because the NuvolaX icon theme seems
to have vanished from the Internet. Ideally all those admin icons could
use a replacement. But that should be a different PR I guess.

adjusted some colors, added a few common extensions for codeblocks

for backwards compatibility

now we no longer limit generation to the extensions in mime types,
because file icons are used in downloadable code blocks, too.

All icons were regenerated with the latest update of the generator.

Made sure all links align as well as possible in a list, giveing them
all the same padding.

Adjusted the template's icons to use 16x16 size and use the additional
space for alignment.

Adjusted some of the default interwiki icons (amazon, skype, paypal).

the new template allows to use a favicon in media/wiki/favicon.ico so we
should support uploading it as well.

This patch reduces the colorspace of GIF files and has optipng
optimization run on PNGs. Changes shouldn't be visible except in
filesizes.
