This is a quick fix in advance of the library being updated to
the latest & PHP7 compatible version.

- replace PHP4 style class constructor function names (based on
  class name) with php 5 __construct()
Also remove some '&' reference operators used with objects
And add some object type hints

This reverts commit c2fbf986.

It is helpful if the pipe is still inserted by the linkwizard when there
is no title. This increases usability for beginners.
